Upcoming Office Closure

The administrative office of the Adoption Medicine Clinic will be closed on January 20th, 2025 in observance of the University of Minnesota holiday. The [email protected] email will not be monitored during this time. Preadoption Medical Reviews may be accepted upon request during this time at the holiday rate. Please contact Dr. Judith Eckerle at [email protected] if you have questions about Preadoption Medical Reviews during this time. 

Preadoption Questions

For general information about preadoption consultation and preadoption medical review services contact us by email for the fastest response at [email protected]. You are also welcome to call our administrative offices at 612-624-1164. If we are not able to take your call, we encourage you to leave a voicemail and we will return your call as soon as possible during regular business hours (8:00 a.m. - 4:30 p.m.).

Please note that our office will be closed New Year's Eve and New Year's Day.

Clinic Fax: 612-365-8001 - Please fax all medically related documents (i.e. lab results, immunizations, etc.) here.

If you would like to fax records to the Adoption Medicine Clinic at Discovery Clinic for your upcoming visit, please fax records to 612-365-8001.

Medical Records and Finance Questions:

To obtain medical records after your visit, including copies of immunization records and lab results, please contact our Medical Records Department.

For billing and insurance questions for the Discovery Clinic contact the Bill Pay and Financial Services department.
