Faculty Annual Reviews

Departments must submit an annual review for every full time, paid, faculty member. The annual review should cover the calendar year.

Please note: annual reviews are not required for part-time (<67 percent effort) faculty. Although not required, departments are encouraged to collect and submit annual reviews for unpaid/without salary faculty. The deadline to submit annual appraisals for all faculty members is June 30.

The Medical School faculty annual review process involves the following items: 

  • Annual review form (either the Summary of Annual Faculty Evaluation (SAFE) form, or the evaluation form from the Works faculty activity system).
  • Form 12 - probationary faculty
  • Form 13 - tenured associate professors every 4 years

Tenure-Track Faculty (Probationary Faculty)

SAFE Form/Works and Form 12

OFA will distribute pre-filled out Form 12s to each department in January. 

Form 12's are shared with [email protected] and should follow the naming convention: Last Name_College abbreviation-Departmental abbreviation Form 12_Academic year (e.g., Smith_MED-ANES Form 12_2024-2025)

Tenured Faculty

SAFE Form/Works and Form 13 (for associate professors every 4 years)

OFA will distribute a list of faculty needing Form 13s to each department in January. 

Non-Tenure Track Faculty (Academic and Clinician tracks)

Note: This includes all paid and unpaid faculty on the academic track and clinician track.

Paid faculty may use the SAFE Form or Works review system (dependent upon departmental processes)

SAFE Forms are shared with [email protected], and should follow the naming convention: "LastName.FirstName.Rank.Track.YEAROFREVIEW."

Reviews completed in the Works system will be saved by OFA.

Adjunct Faculty Annual Activity Reports

Departments submit PDF copies of signed Adjunct Faculty Annual Activity Report in a Google Drive folder shared with [email protected]. Annual Activity Reports should follow the same naming convention: "LastName.FirstName.Rank.Track.YEAROFREVIEW." Reports should be submitted to the department annually by March 15, and departments will submit to the OFA by June 30. 

OFA will send a request to all departments every year, asking for confirmation of adjunct faculty appointment renewals. OFA will communicate if renewals have been approved and if the department can issue a renewal letter.

Med School 7.12 Part 3: Annual Review of Tenured Faculty

Works is an online faculty activity reporting system for recording ongoing activities and accomplishments, such as teaching, research, service, awards, and public and international engagement.

The faculty annual review process in the Works system reduces faculty and administrative staff time and effort in documenting, emailing, and collecting individual review forms from faculty members each year. Works is also a useful tool for promotion as it can automatically generate a CV in the Medical School format.

Faculty enter annual activities and respond to questions for their annual review (formerly the “SAFE” form) on a calendar year basis. Works will automatically route the annual review to faculty members, division directors (as applicable) and department heads. If a department has opted into the Works system for faculty annual reviews, the system will launch in Jan.-Feb. each year.

Faculty will have one place to enter this information on an annual basis. Once the annual review has been completed, the forms will be available to department heads and the Office of Faculty Affairs, eliminating the need to collect/share reports manually.

The timeline for submission will depend on departmental annual review processes and schedules.
