RMC Closing

The following is an important announcement for patients: 

Reproductive Medicine Center (RMC) closed its practice on June 26, 2015. We reached this decision upon learning three of our physicians will move into private practice later this summer. We believe the closing is the right decision for our patients and our staff.

Patients who have frozen embryos, oocytes, sperm or testicular tissue at RMC:
If you are a patient who has frozen embryos, oocytes, sperm or testicular tissue stored at RMC, a certified letter was mailed to you to inform you that your specimens can be transferred to a facility of your choosing, or, after June 21 will be transferred to Fairfax Cryobank in Roseville, Minn.. Fairfax Cryobank has assumed responsibility for long-term storage of all RMC frozen specimens.

Patients must fill out a new storage agreement with Fairfax Cryobank. This form can be found here. Please call Fairfax Cryobank at 651-489-0420 for more information.

About Your Medical Records

Medical records for Reproductive Medicine Center patients are maintained by University of Minnesota Physicians and will continue to be available after the clinic has closed.

When you have selected a new clinic for your care and have an appointment scheduled, please fill out the release of information form and mail it to University of Minnesota Physicians at the address listed on the form. Our staff will process the request and send your medical records to your new clinic. Most requests are handled within one week. Please call 612-884-0650 with questions.

About RMC’s Physicians

  • Dr. Mark Damario has joined the Center for Reproductive Medicine in St. Paul.
  • Dr. Phoebe Leonard has joined the Reproductive Medicine and Infertility Associates in Woodbury
  • Dr. William Phipps will be moving to an out of state practice.
  • Mary Danich, Nurse Practitioner has joined Reproductive Medicine and Infertility Associates in Woodbury.

Andrology (semen analysis) appointments
For patients seeking to make an appointment for a semen analysis (SA), call 612-672-4072. The diagnostic testing lab for SA is located at the same location, different suite number. Address: Riverside Professional Building, 606 24th Ave. S., Suite 525. The lab will start taking appointments on July 6, 2015.

Fertility evaluations at University of Minnesota Health
