Community Health
Family medicine is more than caring for patients within clinic walls—it's caring for patients within the context of their communities. We are actively engaged in caring for communities at home and abroad that are underserved with limited access to health care.
Community Health Goals
- Educate physicians to identify health care needs, remove barriers to care, and promote the health of the communities they serve
- Model excellent health care in clinical and community settings
- Promote primary care research that translates into clinical and community practice
- Recognize communities of special needs and foster mechanisms to provide them with care
Community Health Outreach
- Chronic disease prevention, including diabetes care
- Community education on a variety of health topics
- Mentorship for youths who are underserved with limited access to health care and are interested in healthcare careers (The Ladder)
- Nursing home care and home visits
- Medical care for those who are unhoused:
