Katherine Montag Schafer, PharmD, BCACP, CDCES, is an associate professor in the Department of Family Medicine and Community Health and an adjunct associate professor in the Department of Pharmaceutical Care and Health Systems at the University of Minnesota. She serves as a clinical pharmacist and faculty member with the St. John's Hospital Family Medicine Residency Program, where she provides direct patient care at Phalen Village Family Medicine Clinic while educating family medicine resident physicians and mentoring pharmacy students. Dr. Montag Schafer is a Board Certified Ambulatory Care Pharmacist and Certified Diabetes Care and Education Specialist with clinical expertise in diabetes management, ambulatory care pharmacotherapy, and team-based primary care. She earned her Doctor of Pharmacy degree from North Dakota State University and completed PGY1 Pharmacy Practice and PGY2 Ambulatory Care Pharmacy residencies at Hennepin County Medical Center.
Dr. Montag Schafer's scholarship focuses on improving primary care through community-engaged research, implementation science, and pharmacist integration within interdisciplinary care teams. Her work has centered on developing culturally informed diabetes education in partnership with Minnesota's Hmong community, resulting in innovative digital storytelling, text messaging, and other community-designed educational interventions to improve diabetes self-management. She is committed to translating evidence into practical clinical care while preparing the next generation of family medicine physicians and pharmacists to deliver patient-centered, equitable care.
